the basics

What High Achievers Experience

High performers rarely present with the textbook picture. The output stays intact, so the strain reads as drive rather than a problem, and it goes unaddressed for years. You might recognize some of these:

These are clinical signals, not character flaws. They point to real, treatable conditions that often show up differently in high performers than the textbook describes.

Common Conditions We See in High Achievers

These frequently co-occur. The Sigmund Assessment maps the full picture first, so the plan targets what's actually driving it, not the loudest symptom.

High Functioning Anxiety

High functioning anxiety isn't a formal diagnosis; it's a pattern of a nervous system running at full tilt while your performance stays intact. It surfaces as physical tension, disrupted sleep, and relentless mental rehearsal, often passing as thoroughness for years. Evidence-based treatment, including CBT, produces measurable results on a realistic timeline. See our anxiety treatment page.

Burnout

Burnout builds in high-stakes fields like finance, law, tech, and medicine, where the expectation is to push through. It's chronic occupational stress made physical: exhaustion, cynicism, and a drop in the effectiveness you built your identity on. Treating it means addressing the workload, sleep, and structure alongside the psychological side, together rather than one at a time.

Undiagnosed ADHD

Many high achievers carry ADHD they've masked for years through intelligence, overwork, and anxiety-driven hypervigilance. It shows up as chronic procrastination, time blindness, and working twice as hard for the same output. That isn't a discipline problem; it's a neurodevelopmental one, and accurate evaluation changes how you function. See our ADHD treatment page.

Depression

Depression in high performers often presents as irritability, detachment, and overwork rather than visible sadness. Because it doesn't match the stereotype, it goes unrecognized longer and erodes more career, relationships, and health before anyone names it. Accurate diagnosis and early treatment change that trajectory. See our depression treatment page.

How We Care for High Achievers

Every patient's care is informed by the Sigmund Assessment, a structured evaluation your clinician reviews before your first consultation. It maps your symptom history, what's worked and what hasn't, your biology, your environment, and your goals, so session one starts with a plan instead of paperwork.

From there, one clinician handles all of it from the same chair: therapy, medication when it's indicated, and the lifestyle factors that decide how your brain actually runs, including sleep, exercise, and daily structure. There's no prescriber-and-therapist split and no indefinite, hands-off prescription management. We also name what the surface read misses, the undiagnosed ADHD presenting as anxiety or the masked depression presenting as irritability, because in high performers the obvious symptom is rarely the whole picture. Progress is tracked with validated measures, so you can see what's changing rather than wonder.

Scheduling is built for demanding careers, and your care stays discreet. The practice houses clinicians affiliated with the National Institutes of Health, Columbia University, and New York-Presbyterian Hospital, and you can explore the research behind how we assess and treat. Care is available in person at our Manhattan office and by telehealth anywhere in New York State.

Insurance and Billing

Integrative Psych is an out-of-network practice. You pay at the time of service and receive a monthly superbill to submit to your insurer for potential out-of-network reimbursement. Many PPO plans reimburse a portion, and your insurer is the best source for what your plan covers. To check yours, call the number on your card and ask about out-of-network reimbursement for outpatient psychiatric evaluations, follow-up visits, and therapy.

Frequently Asked Questions

How do I know if it's high functioning anxiety or just stress?

Stress resolves when the stressor does; this doesn't. If you've been running at full tilt for months while your performance holds but your internal experience is dread, exhaustion, or constant self-monitoring, that's a clinical pattern worth evaluating. The Sigmund Assessment maps the full picture before any treatment decision, so you get a clear read, not a label.

Yes. Many adults compensate for ADHD through intelligence, overwork, and hypervigilance, so it looks like success from outside and feels like constant struggle inside. A structured evaluation separates ADHD from anxiety-driven compensation, which matters because the treatment for each is different.

Burnout is a response to chronic occupational stress: exhaustion, cynicism, reduced effectiveness. Depression affects mood, motivation, sleep, and cognition regardless of work context. They overlap often in high performers, and a thorough evaluation distinguishes them so the plan targets what's actually driving the presentation.

Not different clinically, different practically. Sessions are goal-oriented and direct, scheduling is built around demanding work, and your care stays discreet. If past therapy felt unfocused or generic, the issue was usually the match, not the modality.

We are not an emergency treatment clinic. When you book, we match you with the clinician who fits your goals, so you start with the right person rather than the first available. Generally you can get in within a week of contacting us. If your situation feels urgent, call 911 or go to your nearest emergency room. For consultations, call 646-893-8935.

Our office is at 80 8th Ave in Manhattan, where high achievers are seen in person by one clinician for medication and therapy. We also offer telehealth anywhere in New York State, which fits around a demanding schedule.

Crisis Resources: We are not an emergency treatment clinic. If you or a loved one is in crisis, call 911, call or text 988 (Suicide and Crisis Lifeline), or go to the nearest emergency room. Integrative Psych is an outpatient practice and is not equipped to provide crisis intervention.
