What are Schizophrenia & Psychosis
Schizophrenia is a psychiatric condition that affects how a person thinks, perceives reality, and organizes daily life. It usually emerges between late adolescence and the early 30s and is highly heritable, though most people with a family history never develop it. Psychosis itself is not a diagnosis. It's a symptom cluster that can appear in several conditions, and naming the right underlying one is the first real piece of clinical work.
Schizophrenia sits alongside related diagnoses: schizoaffective disorder, brief psychotic disorder, schizophreniform disorder, and substance-induced psychosis. Accurate diagnosis across this spectrum matters because treatment differs meaningfully between them, particularly between schizophrenia and bipolar disorder with psychotic features.
Symptoms typically fall into three groups
Treatable, and too often untreated
U.S. adults lives with schizophrenia or a related psychotic disorder (U.S. estimates range 0.25%–0.64%; NIMH)
people worldwide lives with schizophrenia (WHO)
people with psychosis worldwide don't receive specialist mental health care (WHO)
How Schizophrenia & Psychosis Are Diagnosed
Schizophrenia and related psychotic disorders are diagnosed clinically, through a detailed conversation about your full history, not by a blood test or scan. Your clinician builds a timeline: when symptoms started, how they've changed, what treatment has and hasn't worked, and what your family history looks like. The workup rules out medical mimics, screens for co-occurring depression, and reviews substance use, since cannabis, stimulants, and several other substances can trigger or worsen psychotic symptoms.
Because your clinician reviews the Sigmund Assessment before your first appointment, they already hold the timeline, so your first appointment goes straight to interpretation and a real conversation about what's recommended and why.
A note on scope. Integrative Psych is an outpatient practice. Acute psychosis with safety concerns, severe disorganization, or a first break without prior care typically needs evaluation in an emergency room or inpatient unit first. If you reach out during an acute episode, we'll help coordinate the right level of care and can take over outpatient management once you're stabilized.
Treatment Options for Schizophrenia & Psychosis
Effective care combines medication, therapy, and family support, and the combination outperforms any single piece.
Antipsychotic medication
Antipsychotics are the foundation of treatment. Consistent medication keeps positive symptoms in remission, reduces relapse risk, and protects cognitive and functional recovery. Second-generation antipsychotics like aripiprazole, risperidone, and lurasidone are the typical first-line options, each with a different profile across sedation, metabolic effects, and movement side effects. For people whose symptoms haven't responded to two adequate trials, clozapine is the most effective option for treatment-resistant schizophrenia and has distinct evidence for reducing suicide risk, with ongoing blood monitoring. Long-acting injectables are an option for anyone who prefers not to take a daily pill or has had relapses tied to missed doses.
Supportive therapy
Medication does the core work; therapy does work medication can't. Supportive psychotherapy targets sleep, routine, stress, adherence, and early warning signs of relapse. Cognitive behavioral therapy for psychosis helps people reduce the impact of distressing beliefs and voices, and cognitive remediation rebuilds the attention and memory that limit work and school.
Family education
Schizophrenia is one of the few psychiatric conditions where family involvement consistently improves outcomes, lowering relapse and hospitalization. We cover what the diagnosis means, the early signs of relapse, adherence support, and crisis planning. Family sessions are scheduled alongside individual care when you and your family are ready.
How We Treat Schizophrenia & Psychosis at Integrative Psych
Care here is delivered by one clinician in a single plan: antipsychotic medication management, supportive therapy, and family education, coordinated from the start. There's no prescriber-and-therapist split and no repeating your history to a new face every visit. When one person sees the early shift, the medication adjustment happens the same week instead of weeks later, which is exactly where fragmented care tends to lose ground and relapse risk climbs.
Our clinicians are board-certified psychiatrists and psychiatric nurse practitioners with deep psychopharmacology training, and the practice houses clinicians affiliated with the National Institutes of Health, Columbia University, and New York-Presbyterian Hospital. The way we assess and manage psychotic disorders is informed by academic research, and you can explore the research if you want to look deeper. Care is available in person at our Manhattan office, and by telehealth anywhere in New York State.
